In reference to the exclusion of critically ill patients from clinical trials, we agree with Drs Arya and Kulkarni that “opinion and perception of patients who do not participate need to be detailed and systematically analyzed.” However, systematic exclusion of critically ill patients from clinical trial participation would be more likely to reduce the outcome for nonparticipants, thereby artificially increasing the clinical trial effect.
