In Reply.—We are distressed that Dr Poulton and Ms Grady have chosen to lay blame for potential medication errors on the misuse of a relatively simple and straightforward medical device. The slider of the EMERGI-DOSE guide can be removed and reversed, but each slide of this inner slide is clearly marked as to its appropriate utilization, ie, adult or pediatric, as are the thumb tags at the ends of the slider. Furthermore, when the slider is inserted in a reversed fashion the numbers do not line up exactly and are somewhat off-centered.
As with any medical device, proper handling and correct utilization are mandatory to avoid potential complications. Such a warning is clearly stated in the literature distributed with the guide, and this product is only marketed to health personnel in emergency and critical care settings—individuals presumably familiar with these medications.
