A 12-year-old boy had nephrocalcinosis and hypertension associated with primary hyperparathyroidism due to parathyroid adenoma. Nephrocalcinosis was discovered when a left renal biopsy was done during the course of surgical exploration for possible renal vein thrombosis. The hypertensive episode was amenable to drug therapy. Nephrocalcinosis and hypertension seem to be rare manifestations of juvenile primary hyperparathyroidism.
