The pulmonary angiographic findings are presented in a patient with infantile lobar emphysema prior to the full expansion of the emphysematous lobe. There was central displacement and compression of vessels of the involved lobe, concomitant displacement and compression of adjacent nonemphysematous structures, and anomalous venous drainage of the emphysematous lobe. The angiographic appearance of the vessels of the involved lobe in this patient differs from that of previously published reports in which the vascular morphological appearance was observed in the hyperexpanded lobe. Pulmonary angiography may be useful in demonstrating the site of the abnormality in patients with infantile lobar emphysema prior to the appearance of the classical roentgenographic manifestations.
