To the Editor: I agree that the statement, "cultures were obtained at the end of therapy" left the reader questioning the time with respect to the end of therapy when these cultures were taken. I would like to answer this question by stating that the cultures were taken prior to therapy, at 4 to 7 days after initiation of therapy, at 12 to 14 days at the end of therapy, and again at 21 days and at 31 days. When these repeated cultures were read as negative for α-streptococcus, the patient was considered cured, and thus reported.
The regularity with which the patient took medication can only be proven by our results, as reported in the article. As to Dr. Mortimer's statement, "in relation to a disease that is self-limited and usually shows improvement within 48 hours without therapy," this has not been our feeling.
