Follow-up care for the epileptic patient has been greatly simplified for me by the form on the next page.
At each visit the date is recorded at the top of a succeeding vertical column.
The medication last tried is then entered (in milligrams) in that column opposite its name.
A point is then placed in the middle of the box in that column opposite the most correct seizure frequency in the last trial period (i.e., one seizure in two months would be a point opposite 6/yr.).
The points are then connected by the designated color or type of line.
If the patient has two or more seizure types, as grand mal and petit mal, both are graphed on the same sheet.
Idiosyncracy is marked by red asterisk and a number and described under "Comment" or on progress note sheet.
